THOUSANDS of victims of genital mutilation live in NSW and experts say doctors are untrained in how to treat them, with many suffering from childbirth complications as a result of their condition.
With an increased intake of refugees from countries in Africa, the Middle East and south-east Asia, more women with genital mutilation are living in Australia, with most in NSW.
It has prompted the state Health Department to develop the first guidelines for clinicians treating women affected by the barbaric procedure, which involves partial or total
removal of the genitalia.
"We didn't want to be in that situation ever again."An obstetrician specialising in the care of women with genital mutilation, Greg Jenkins, said it was often not identified by doctors until women were admitted to hospital in
labour.
''That's really too late and it needs to be recognised earlier in their pregnancy so the opportunity for counselling and management is there,'' Dr Jenkins said. ''The difficulty is, in
the country where they come from, genital mutilation is common so midwives there assume women have had it done and know what to do.''
Dr Jenkins works at Auburn Hospital, the only hospital with dedicated guidelines on how to care for mutilation victims. His program requires at least one senior midwife
experienced in female genital mutilation and childbirth to be on hand at every shift. Women are assigned a midwife who they see exclusively throughout their pregnancy, with an interpreter available for those with little English.
''What prompted our guidelines was a 16-year-old girl who came into hospital in labour in 1993,'' Dr Jenkins said.
''She had quite a severe form of genital mutilation and the staff, who had never seen a case before, didn't know how to deal with it. She ended up with a nasty tear, and when
the operating theatre consultant came in they didn't know what to do about it. We didn't want to be in that situation ever again.''
Dr Jenkins said he had not been consulted about the development of the clinical guidelines but said he supported anything that would help improve treatment. While they will not
be mandatory, a NSW Health spokeswoman said hospitals would require ''very sound reasons'' for not implementing them.
Without specialised care, women with genital mutilation are at higher risk of birth complications, including foetal distress, hemorrhage and tearing.
One woman, who did not want to be identified, said hospital staff were uncertain of what to do when she was in labour with her second child and they saw her vagina had been
almost closed through mutilation.

''[So] they cut [me],'' she said.
''But they didn't stitch the incision. After [I went home], I felt some of my genital tissue was sticking out and that the area was still opened wide.''
She went to see a doctor, who did nothing. ''So I lived with that condition until I had my third child,'' she said.
''The baby was big and during that labour the incision was enlarged more and I had complications. I was bleeding for 15 minutes and the midwife was anxious; she didn't realise what was happening to me.''
The draft guidelines will be released next month, when they will be considered by Local Health Districts before being implemented in early to mid-2013.

Another facet of Opposition Leader Tony Abbott's student activist days has surfaced with a speaker at a pro-Palestine rally in Canberra declaring him a "raving Zionist".
That gathering debated the proposal for a delegation of the Palestine Liberation Organisation (PLO) to visit Australia.
At the time the PLO group was classified a terrorist organisation, although its offshoot, Fatah, which now rules the West Bank, is more favourably regarded than Hamas, which rules Gaza.
Ms Sattler said the AUS vote backed hearing from the PLO.
"Guess who was on the other side at that time - Tony Abbott," she said.
"That was my first introduction to Tony Abbott and I've been his enemy ever since."
Ms Sattler said Mr Abbott had no Jewish heritage, although she did.
"He is a raving Zionist and has been since he was about 18 or 19. I don't understand why he is, but he is," she said.
Ms Sattler said those who backed the PLO visit were beaten with rolled up newspapers and umbrellas and spat upon during the course of the conference.
She said this made a big impact on her as a 17-year-old activist.
"Thanks to Tony, I have been an avowed Palestine supporter ever since," she said.
In the 1970s, Mr Abbott was a student activist strongly opposed to left domination of student politics.
Earlier this year, journalist David Marr recounted a 1977 incident in which Mr Abbott allegedly sought to intimidate a female student who beat him in a university election by punching a wall on either side of her head. Mr Abbott has repeatedly denied this happened.
Who IS Kim Sattler, and like her fellow Socialist Labor Emilys list "sista", Australian PM, Julia Gillard can you believe a word she or PM Julia say about anything ?

SHE'S the Labor true believer the Prime Minister's press adviser called when he wanted to politically damage Tony Abbott.
Unions ACT secretary Kim Sattler, outed yesterday as staffer Tony Hodges' contact at the Tent Embassy protest, was always going to rally to the cause.
A Left-wing activist who fronted at a pro-carbon tax event last year and who sits on the board of the National Museum of Labour, Ms Sattler helps oversee 24 ACT unions with more than 30,000 members.
In a profile for a Canberra networking group she lists union darlings Joan Kirner, Sharon Burrow and Ged Kearney as her inspiration.
But the Prime Minister, a former union head, denied links with Ms Sattler as it emerged Mr Hodges gave her information about Tony Abbott's location moments before the ugly Australia Day riot.
Ms Gillard yesterday said she did not know Ms Sattler, who was previously not widely known outside her ACT stamping ground.
An image added to Ms Sattler's Facebook page in 2007 shows her with Ms Gillard apparently at a function for Emily's List - a Left-leaning group promoting progressive women's political careers.
Another image shows her with former Victorian premier Joan Kirner, while one picture, believed to have been taken overseas, appears to show Ms Sattler with a gun.
Yesterday morning, Ms Sattler berated Mr Abbott through the social network, saying he played a role in the violence.
"Tony Abbot is like your typical bar-room brawler, who starts a fight and then disappears like a coward when it is in full swing," she wrote.

Ms Sattler yesterday confirmed she called the Australian Federal Police into her office at 1.30pm on Friday and informed them she had been the one who passed on the location of Mr Abbott to protesters.
On Thursday, Ms Sattler posted a photo, taken on her iPhone, of the crowd engulfing the medal ceremony and added the comment, "Tony Abbott just announced the Tent Embassy should be closed down and a huge crowd from the Embassy went to greet him and he had to be rushed away with a police escort!"
The Facebook profile was suspended yesterday after Ms Gillard named Ms Sattler as the Tent Embassy contact of her former adviser (Hodges).
Ms Gillard said it was "very very likely" she had been at the same events in Canberra as Ms Sattler, but flatly denied a connection.
What's the deal with Socialist's,Socialist "Wimmin" in particular,and their propensity for not been able to tell the truth of just about anything?

PRO-Palestine broadcaster ABC skips the facts when it covers the Gaza conflict.
Lateline presenter Emma Alberici didn’t mind giving the evil Hamas terrorists a leg-up when she permitted Arab propagandist Rami Khouri to take over her show almost uninterrupted.
Khouri, from the American University of Beirut’s Institute of Public Policy and International Affairs, told blatant untruths - and was unchallenged.
How about this whopper: “The Arabs have given the Israelis a peace plan and accepted to live with a Israeli state as it is now, predominantly a Jewish majority Israeli state.”
Totally false as anyone with the slightest interest in Middle East peace would know. The Arabs have not presented any peace plan that would support a predominantly Jewish majority state. Never have since
Israel was created by the UN in 1948 only to face continuing Arab hostility.
That doesn’t bother the ABC though.
“We have given them a formal peace plan,” Khouri baldly stated. Total nonsense.

They were landing not just in Israeli territory, but in areas that are populated. And there’s no country on Earth that would tolerate missiles raining down on its citizens from outside its borders.”
“If we’re serious about wanting to resolve this situation and create a genuine peace process, it starts with no more missiles being fired into Israel’s territory, and that then gives us the space to try to deal with these longstanding conflicts that exist.”
Britain’s Foreign Secretary William Hague said: “We call on Hamas again to stop the rocket attacks on Israel. It is Hamas that bears, as, as I’ve said before, the principle responsibility for starting all of this and we would like to see an agreed cease fire, an essential component of which is an end to those rocket attacks.”
How did the ABC miss these world leaders’ views on the attacks, how did they get it so wrong?
No-one should be in any doubt about the real nature of Hamas and its military wing, the Al-Qassam Brigades.
This is the message Hamas is broadcasting on its Al-Aqsa TV: “From the Al-Qassam Brigades to the Zionist soldiers: The Al-Qassam Brigades love death more than you love life.”
This is the standard taunt of Islamist murderers worldwide.
Up to Monday, more than 1,900 rockets and mortars had been fired at Israeli civilians from Gaza this year. More than three million Israelis live within range of rocket and mortar attacks.
In the area closest to the Gaza border where most rocket attacks occur there are more than 90 schools and 61 kindergartens.
All kindergartens and schools remain closed within a 40-kilometer radius of the Gaza border.
All residents within this area are urged to remain within a maximum 15 seconds from a bomb shelter or protected area – the time residents have to reach shelter once radar detects incoming projectiles and triggers air raid sirens.
Not for the first time, Israel is treating Gaza patients in its hospitals - 19 hospitals are located in the range of Hamas’ rockets.
By midday Monday, Hamas had launched at least 45 rockets into Israel.
More than 880 rockets and mortars have been fired from Gaza since the beginning of Operation Pillar of Defense.
Yet our ABC finds it more convenient to repeat the propaganda of a terrorist regime which is holding reporters hostage and hiding its armed forces within a civilian population.

Here's what really happened.
Julia Gillard was asked in her formal interview by Peter Gordon whether or not she knew anything about the Slater and Gordon firm providing a mortgage to finance the purchase of 1/85 Kerr Street Fitzroy.
The PM is a bit of a galah beside being a big liar. She says she didn't know about the mortgage.
She thought that the only thing that The Age or the 7.30 show on the ABC had on her was the note from the Commonwealth Bank about the insurance on the property. But that's not all. She also made other "random" enquiries about a mortgage she knew nothing about, and those random enquiries found their way into the Ralph Edwin Blewitt mortgage file.
Here's what she said to Peter Gordon on 11 September 1995.
Peter Gordon: “Were you aware at any time that the balance of the funds to make up the capital was to be provided by contributory mortgage of which Jonathan Rothfield (a Slater & Gordon partner) was trustee?”
Julia Gillard: “I don’t, I don’t think I knew that at the time, where the source of funds was. It’s subsequently been raised with me that that was done through the Slater & Gordon mortgage register but I didn’t have any recollection of that.”
The Age and the ABC's 7.30 program relied on the letter from the Commonwealth Bank to show that Gillard knew all about the mortgage.
And so her improbable response to that letter from the Comm Bank, addressed to her. One again she tries to drop the patsy Ralph Blewitt in the cack.
But wait Julia, there's more!
The dodgy Power of Attorney was improperly used in an application to the Slater and Gordon mortgage provider to procure a $150,000 mortgage in Ralph Edwin Blewitt's name. He would be liable for the debt.
But he knew nothing. Here is the letter of offer about the mortgage. Keen observers will note to whom it is addressed.
It goes on for a few pages, I've reproduced the first page above. The next page includes this paragraph.
At some point, Julia spoke with someone who cared about whether or not he would be charged the higher rate of interest. But keep in mind that Julia Gillard knew nothing about the mortgage. Just out of the blue, Julia asked her PA to make enquiries, and Julia's PA's note found its way into the mortgage file.
And Olive Brosnahan wrote this note to Gillard about the mortgage that Gillard didn't know about.
And it wasn't enough for Julia to just get that information about the mortgage she knew nothing about verbally. Or even hand-written. It was typed out. Nicely. In a way that you could take home and show to your lover who was enquiring about the mortgage he was acquiring. But Julia knew nothing about that. Just ask her.
I am running a very unsophisticated blog on a laptop and an iPhone. Just imagine what information about the trustworthiness of Julia Eileen Gillard has been uncovered by the security agencies from countries that Australia deals with.

Prime Minister Julia Gillard today refuted claims of impropriety during her time as a lawyer at the firm Slater and Gordon and attacked those making the claims against her.

LEIGH SALES, PRESENTER: For months, a vicious online smear campaign has been questioning Julia Gillard's conduct as a lawyer 17 years ago. It's spilled over into the national news media and today the Prime Minister declared enough is enough. In a fiery defence of her time at Slater & Gordon, she said News Limited stories about her involvement in the creation of a union fund were defamatory and wrong.
Political editor Chris Uhlmann reports on an unusual day in federal politics.
CHRIS UHLMANN, REPORTER: It began as an announcement about lifting the refugee intake.
It was punctuated by an appalling security breach when an intruder got within striking distance of the Prime Minister.
JOURNALIST: How did you get in?
INTRUDER: No, just got in through the public entrance.
JOURNALIST II: Prime Minister, something very odd just occurred. What have you been handed there?
JULIA GILLARD, PRIME MINISTER: Oh, Mark, I wish could help you, but I’ve got no idea.
CHRIS UHLMANN: It morphed into a defiant challenge by Julia Gillard.
JULIA GILLARD: I'm answering your questions now. If you've got questions, put ‘em.
CHRIS UHLMANN: And over one hour and 13 minutes it became one of the most extraordinary press conferences of recent years as Julia Gillard acted to end questions over her character that reach back 20 years.
JULIA GILLARD: None of this is new. I am standing in front of you now as a 50 year old woman. Have I learned a few things across my lifetime? Yes, I have.
CHRIS UHLMANN: As a 30 year old woman the Prime Minister was a partner in law firm Slater & Gordon and had begun a relationship with an up-and-coming Australian Workers’ Union official, Bruce Wilson. It would end in acrimony in 1995 amid allegations by the AWU that Bruce Wilson and fellow official Ralph Blewett had misappropriated union funds. The Prime Minister has always said she was unaware of what the two were doing.
JULIA GILLARD: Once I became aware that I had been deceived about a series of matters, I ended my relationship with Mr Wilson.
CHRIS UHLMANN: No charges were ever laid against Bruce Wilson or Ralph Blewett, to the great disappointment to the union and police, even though an association they set up in WA with a noble aim of achieving safe workplaces used money from building companies for their own benefit, including buying a house in Victoria.
JULIA GILLARD: I did not at the time understand that any funds from any other source would be used to support the purchase, that is funds from the association or any other accounts related to the union.
CHRIS UHLMANN: At the heart of the allegations against the Prime Minister is that she did the legal legwork to set up the association, that she never kept a file on it at her law firm and that she described it as a “slush fund” when questioned about it by the senior part partner.
JOURNALIST III: Did you help to draft the document that you knew was false because the purpose of the association was fundraising for a union election, not workplace safety, as stated in that document?
JULIA GILLARD: My understanding of the purpose of this association was to support the re-election of union officials who would run a campaign saying that they wanted re election because they were committed to reforming workplaces in a certain way, to increasing occupational health and safety, to improving the conditions of the members of the union.
CHRIS UHLMANN: On the weekend the Prime Minister said she wouldn't dignify any of these allegations with a response, despite the fact that The Australian newspaper had been running hard with them.
JULIA GILLARD (Sunday, Sky News): Delving into matters 17 years ago for what purpose? If you've got an allegation I did something wrong, put it. If you can't put it, why are we talking about this?
CHRIS UHLMANN: Today The Australian was forced to apologise for reporting that the Prime Minister had set up a trust fund for Bruce Wilson and Julia Gillard decided that attack was the best form of defence.
JULIA GILLARD: These are defamatory allegations and they are wrong.
CHRIS UHLMANN: This rebooting of ancient charges began as an internet campaign and one of its torchbearers is cartoonist Larry Pickering. His base attacks on the Prime Minister are among the many reasons why much of mainstream media hasn't revisited this story.
But The Australian's recent investigation did raise new information and new questions, like why Julia Gillard never opened a file on the association. She says it was routine to provide free advice.
JULIA GILLARD: With the benefit of hindsight, of course it would have been better that I had opened the file on the system, but at the time it did not strike me as a matter of particular significance.
CHRIS UHLMANN: What is significant is who was fashioning the bullets being fired at the Prime Minister. As you would expect, fingers are being pointed at the Opposition, but to date its attack’s been muted, and whenever it does come up, a more intriguing trail is raised.
TONY ABBOTT, OPPOSITION LEADER (Saturday): I think there are questions that she does need to address. Let's not forget that these issues were most recently raised in the Parliament not by the Coalition, but by a member of her own cabinet, Robert McClelland. I think there are questions that the Prime Minister does need to answer.
CHRIS UHLMANN: Only aficionados of this ancient case could have spotted an attack on the Prime Minister buried in this June speech by the former Attorney General.
ROBERT MCCLELLAND, ALP BACKBENCHER (22 June): Indeed I know the Prime Minister is quite familiar with this area of the law as lawyers in the mid-1990s were involved in a matter representing opposing clients. Indeed, my involvement in that matter has coloured much of my thinking in this area.
CHRIS UHLMANN: So as with so many things in this parliament, there's a deeper story. The recent attacks on the Prime Minister cannot be divorced from the deep desire some in her own party have to see her deposed and the people writing these stories are being offered assistance from inside Labor's ranks. The Prime Minister hopes today draws a line under the allegations.
JULIA GILLARD: Will the misogynists and the nut jobs on the internet continue to circulate them? Yes, they will. And it wouldn't matter what I said and it wouldn't matter what documents were produced and it wouldn't matter what anybody else said, they will pursue this claim for motivations of their own which are malicious and not in any way associated with the facts.
In terms of the conduct of more mainstream media, well you are in a better position to answer that than me.
But I've been on my feet now for, what? - I can't quite recall; 50 minutes, something like that, taking every question that the journalist elite of this country have got for me. If that doesn't end the matter, then, with respect, I don't know what would.
CHRIS UHLMANN: Malicious motivations aren’t confined to the internet and Julia Gillard's hope that this marks the end of the matter might be optimistic. But the Prime Minister was challenged to answer lingering questions and she did that today and she did it well.
She has, in the estimation of her former law firm and its former senior partner, done no wrong and her performance today was one of the best she's given in a long time.
But the rise of the embittered citizen journalist raises other questions for those who would seek to serve as politicians.
JULIA GILLARD: Yes, it does worry me that that's where politics has got to. That things that are demonstrably untrue, indeed, absurd, are circulated and recirculated and recirculated and somehow, at least in some section of the population, manage to worm their way in to become the orthodoxy.
CHRIS UHLMANN: The Prime Minister has good reason to worry.
